Baal Perazim is a 501(c)(3) organization that operates in part, by in-kind donations. We believe in giving back- in whatever way possible. We offer two charity programs: Charity Care and Men’s Health Ministry to assist in closing the gap in men’s health.

What is Baal Perazim’s Charity Care Program?

Baal Perazim’s Men Charity Care Program is the first of its kind. The overarching goal of the Charity Care Program is to address the health disparities among men by offering safety-net wellness care, regardless of their ability to pay. Services are offered on a self-pay sliding fee scale.

Am I eligible for Baal Perazim’s Charity Care Program?

  • Have eligibility phone call
  • Have no public or private health insurance
  • Not be Medicaid eligible
  • Meet income guidelines (see chart below)
  • Provide photo identification
  • Provide income verification upon request
  • Provide proof of residency

Is Charity Care offered for all three levels of services?

  • No, the Charity Care Program is limited to the wellness in-office visit only.

Is Baal Perazim’s Charity Care Program a church or government clinic?

  • No, Baal Perazim is a private charity men health practice that specializes in men health-related issues.

What ages can apply for the Charity Care Program?

  • Baal Perazim believes that wellness care is a lifelong obligation, we accept clients starting at the age of 11 (adolescents) to geriatrics.

How do I schedule an appointment for the Charity Care Program?

  • Charity Care Program appointments are on a first-come, first-served basis. If all appointments are filled for a given day, you will be placed in the next available slot.

What if I have an urgent need?

  • Baal Perazim uses its best medical judgment to decide who should be seen first. Every effort will be made to triage your needs.

What if I need labs, medications or other services?

  • Baal Perazim provides the initial and follow up wellness visits. Through our partnerships, we have access to low cost medications. We order low-price medications and use coupons for common medications that meets your treatment plan. If Lab, tests, and x-rays are needed, Baal Perazim will help you navigate to community resources that will assist you with these services. We do not cover specialist visits, operations, or hospital care for our uninsured clients.

Do you offer weekends and evening appointments for the Charity Care Program?

Absolutely!  flexibility in scheduling is one of Baal’s intervention to reduce and ultimately eliminate men’s health disparities. Men are the cornerstone of the family unit and often time work hours and other social determinant of health prevents them from receiving care in the traditional 9-5 window. Baal understand that the lack of flexibility in traditional health system has been a lurking barrier for men.

Wait- Have qualifying phone call or email. Have income information and application ready.

Get Approval- Approval/Rejection will be given upon the completion of this call or email.  If approved appointment will be made during call/email.

Appointment- Be on time for your appointment. Bring in all medication that you are taking.

Heart Disease and Diabetes Prevention

Baal Perazim means “breakthrough”

Baal Perazim means “breakthrough.” Men’s Health Ministry is FREE onsite service (at your location) offered to churches and community groups. Upon request, Men Health Ministry will come to your location and provide men congregants/participants with free Heart and Diabetes Screening.

WHY HEART DISEASE AND DIABETES?

Heart disease is the leading cause of death among men in the United States. Cardiovascular disease includes:

  • Heart Attack,
  • Stroke,
  • Heart Failure, and
  • Valve Problems.

African American men have a 44% higher rate of heart disease and a 70% higher risk of heart failure than white men.

According to the US Department of Health and Human Services, National Diabetes and Digestive and Kidney Disease; men are more likely (36.6 percent) than women (29.3 percent) to be diagnosed with prediabetes.
